Case 3

A 57 yo male patient was admitted to the hospital due to repetitive short episodes of chest pain during everyday activity and at rest, dyspnea and legs swelling. He had no previous history of myocardial infarction or longstanding episodes of chest pain. Transthoracic echocardiography (TTE) was done.

Video 3-1 shows the 4-chamber apical view. Video 3-2 shows 2-chamber apical view. Video 3-3 shows 3-chamber apical view. Video 3-4 shows the parasternal long-axis view.

Video 3-5 shows parasternal short-axis view. TTE revealed left ventricle ejection fraction (LV EF) – 30%, LV end-diastolic volume (LV EDV)- 250 ml and mild mitral regurgitation. Coronary angiography revealed multivessel disease, SYNTHAX score- 43 (figures 3-6, 3-7, 3-8 show left coronary artery; figure 3-9 shows the right coronary artery).
